Objective:Ankylosingspondylitis(AS)isachronicinflammatorydiseaseaffectingthespineandpelvisofyoungadults.OntheHLA-B27geneticbackground,theoccurrenceofASisinfluencedbytheintestinalmicrobiota.Thegoalofourstudywastotestwhetherbreastfeeding,whichinfluencesmicrobiota,canpreventthedevelopmentofAS.

Methods:First,patientswithHLA-B27-positiveASfulfillingthemodifiedNewYorkcriteriawererecruitedintheDepartmentofRheumatology,SteMargueritehospitalinMarseilles.Atotalofhealthysiblingswerealsorecruitedtomakeupacontrolgroupwithinthesamefamilies.Second,healthycontrols,andpatientswithrheumatoidarthritisandtheirsiblingswererecruited.Thedatacollectedwereage,gender,numberofbrothersandsisters,ageatdiseaseonset,typeanddurationoffeeding(breastorbottle).

Results:PatientswithAShadbeenbreastfedlessoftenthanhealthycontrols.Infamilieswherechildrenwerebreastfed,thepatientswithASwerelessoftenbreastfedthantheirhealthysiblings(57%vs72%),givinganORforASonsetof0.53(95%CI(0.36to0.77),pvalue=0.).BreastfeedingreducedfamilialprevalenceofAS.ThefrequencyofbreastfeedingwassimilarintheASsiblingsandintheunrelatedcontrols.However,patientswithASwerelessoftenbreastfed
